decoder is DXVA/H.264

MPC-HC Built-in?
It should be just fine, I am not sure with the cause.
If you are not using the latest version, try to update it:

This is not very old, it should not matter.
You may try to remove the other packs and install the K-Lite Mega.

I just install corecodec and DIVX/Xvid.
